Admissions - How to make a referralWolverine Human Services offers a wide variety of residential and community based programs designed to provide cost effective, evidence based treatment. Each of our programs has specific criteria for placement. Our programs have been developed to serve clients with various treatment needs, utilizing community based, non-secure, secure and abuse/neglect (non-secure) facilities. Wolverine is under contract with the Michigan Department of Human Services, all Wayne County Care Management Organizations and many juvenile courts throughout the state. Referrals are accepted from any county in the state of Michigan and out-of-state referrals will be considered on a case-by-case basis. Additionally, Wolverine programs are open to private referrals. If there is a youth or client in need of residential treatment, please use contact information below. The intake specialist will provide a pre-screening to determine if the youth's needs can be met within Wolverine's continuum of care. If there appears to be a match between services and needs, intake will request that you print out the application form (use this as a hyperlink to our application) and complete it. The application also details what documents WHS will need in order to facilitate the referral. The completed application and documents can be faxed or scanned/emailed to the intake specialist. Intake will contact the referring worker or guardian to schedule and interview/assessment and admission for the client. Typically, Wolverine intake staff will travel to the location of the youth for the interview. WHS will ensure that the youth is admitted into treatment in a timely manner.
