Client Qualifications

Must be between 18-20 years of age |

Have an open foster care case or have exited foster care at or after 18 years of age |

Be referred by the Department of Health and Human Services (DHHS) or from private agencies with the approval of DHHS. |

Complete an application form and questionnaire |

Sign a contract agreeing to abide by the program policy and expectations |

Have or agree to a living arrangement that is safe and secure, according to agency standards |

Assist client in locating community resources to address employment, education and medical needs |

Process of Becoming a YAVFC Client
